You may have questions about everything from registration to nutrition. Please ask any of the workers for assistance with your specific questions and we will make sure to direct you in the right direction.
For Our Visitors
As a family member or friend, your help with our patients is imperative to quick healing, the birth of a child, or simply helping your loved one in and out of the car. If you have questions on where to park, which door to come in, or anything else, please do not hesitate to call our Patient Support Center at 606-526-9005 or 888-883-9005.
You are the reason why we do what we do. We want to meet as many needs as possible with our team. Our Patient Support Services is where you can find your medical records, schedule an appointment, ask questions, or anything else you can think of!
Patients and visitors at Grace Health play an active role in preventing infection. We have lots of information and resources (posters on the wall, handouts, etc.) that are aimed to prevent infections from spreading. This will hold us accountable and help you be more informed how you can keep those around you safe as well.
Washing your hands is among the best ways to stop the spread of germs. We have a conveniently located restroom at each of our locations for you to do so!
There are several ways to help prevent the flu, including:
- Washing your hands
- Cover your mouth and nose when you sneeze or cough
- Avoid touching your eyes, nose, or mouth
- Stay home when you are feeling sick
Special precautions may be taken because of a known or suspected infection. It is very important to protect all of our patients from infection. We need to use special practices to stop the spread of some infections. The information sheet below tells you about such special precautions.
Hardship Application
Grace Health is a Federally Qualified Healthcare Center and as such, we provide the opportunity to apply for a discount in your payment through our Hardship Application. Please download the below document, print it out, fill in all of the applicable spaces, and mail it in to the address at the bottom of the form. Your application for a hardship will be reviewed and feedback on our ability to reduce or waive your patient responsibility will be communicated with you in a timely manner.

Appointments
Grace Health operates on an appointment basis, with “Same Day” slots made available to accommodate patients who need to be seen the same day. To strengthen continuity of care, patients are scheduled with the same medical provider whenever possible. Call during normal business hours to schedule an appointment at a specific location.
