Overview

Grace Health Community Programs serves as a beacon of compassion and care, dedicated to improving the health and well-being of communities across our six service area counties through a variety of impactful initiatives.

Our Community Outreach team works tirelessly to address the diverse health and wellness needs of those we serve. From hosting health fairs and enrollment events to forming strategic partnerships with local organizations, our outreach efforts aim to provide essential resources and support where it’s needed most.

With a deep passion for helping people and solving problems, our Outreach Department is committed to expanding healthcare access throughout Southeastern Kentucky. We take pride in engaging directly with the community, connecting individuals to high-quality healthcare services, and ensuring that everyone has the opportunity to access the coverage and care they deserve.

EligibilityHow can I find out if I am eligible for Medicaid, KCHIP, payment assistance, or special discounts?
You can complete an application with a kynector to find out if you or anyone in your household is eligible.
kynectorsWhat is a kynector?
kynectors are individuals in the community who can help you apply for Medicaid/KCHIP, Kentucky Integrated Health Insurance Premium Payment (KI-HIPP) program, and Qualified Health. Plan, SNAP, and Child Care Assistance Program benefits.
Connecting with a kynectorWhere can I complete an application for health insurance?
There is a kynector at all of our main Grace Health locations. Click above to find contact information for the one in your area. If you are a student, faculty, or staff member at University of the Cumberlands, you can contact any kynector for additional information.
Enrollment PeriodWhen can I apply for health insurance?
The open enrollment period (OEP) is between November 1 – January 15 each year.
Outside Open Enrollment Period (OEP)Can I apply for coverage if it isn't during the OEP?
Outside the annual OEP, you can get coverage or change plans if you have a life change or income that qualifies for a special enrollment period (SEP) or you qualify for Medicaid or the Health Insurance Program (CHIP).
Special Enrollment Period (SEP)What is a special enrollment period?
A special enrollment period (SEP) is a time outside the annual OEP when you can enroll in health coverage. You might qualify for an SEP if you’ve had certain life events, like losing health coverage or having a baby.
